Permedica S.p.a is careful to respect the Privacy of its customers and visitors to the Site. Therefore, before you provide your information, please read the information below that explains who we are, why we collect your information, how we will use it, and reminds you of your rights.

(A) THE DATA CONTROLLER:
Permedica S.p.a. is the Data Controller; in fact, it is the company that collects and processes your personal data and, pursuant to Article 13 of Legislative Decree No. 196 of June 30, 2003 “Code on the Protection of Personal Data” (hereinafter the “Code”), provides you with the following information.

The data you provide by filling out the electronic form below will be used for:
- Permedica Dental site registration (www.permedicadental.us);
- Allow you to proceed with the acquisition of online documents reserved exclusively for professionals
- Receive detailed information at your specific request
(C) MANDATORY DATA AND OPTIONAL DATA:
We ask you to fill out this form by providing all the data requested as “mandatory” (*) because it is necessary for the completion of the informatvain case of non-completion or incomplete completion it will not be possible to complete the registration and proceed with the download of documents or not be able to send your request. Failure to complete the data indicated as “optional” will not result in any consequences.
(D) HOW WE PROCESS YOUR DATA:
Your data will be processed in accordance with applicable law and, in any case, in such a way as to ensure its security, confidentiality, and to prevent its unauthorized disclosure or use, alteration or destruction.

(E) TO WHOM WE COMMUNICATE THE DATA:
The data will not be disseminated and may come to the attention of companies that on our behalf. These individuals will act as external data processors in accordance with Article 29 of the Code: the names of all external data processors may be requested by means of a communication to be sent to the addresses below.
Data may also be disclosed to lending institutions (to process payment for your purchases).
Your data may come to the attention of the employees and managers of the Marketing, Information Technology, Customer Care Departments of Permedica S.p.a.
(F) YOUR RIGHTS:
By sending a communication to Permedica S.p.a. at e-mail: info@permedica.it you may exercise your rights under Article 7 of the Code, including:
1) Obtain, without delay, confirmation of the existence or non-existence of personal data concerning you
2) Getting:
2a ) the indication of the origin of the personal data, the purposes and methods of processing, the logic of the processing carried out with the aid of electronic instruments, the identification details of the owner or those responsible, the subjects or categories of subjects to whom the data may be communicated or who may become aware of them as managers or appointees; the updating, rectification or, if interested, the integration of the data;
2b) the cancellation, transformation into anonymous form or blocking of data processed in violation of the law;
